Glossopharyngeal Neuralgia

Glossopharyngeal Neuralgia is very rare as it rates 1/100 of trigeminal neuralgia.
It is sometimes difficult to distinguish from trigeminal neuralgia if the area of pain only restricted inside the mouth.
Sharp pain when swallowing is a typical symptom of this disease, which severely disturb daily food intake.
The pain may be accompanied by loss of consciousness.
Carbamazepine is effective to reduce this pain, however, curative treatment is microvascular decompression for the artery running on the glossopharyngeal nerve.
The surgery is quite similar with that of hemifacial spasm, which needs to observe a little lower than the facial nerve.

3D Image
Glossopharyngeal nerve (light blue) was pinched by two culprit arteries on both side of the nerve (red and pink).
3D bone image guides the appropriate course for the glossopharyngeal nerve.

Operative view.
Glossopharyngeal nerve was compressed as shown preoperative image.
The patientfs pain was totally relieved after microvascular decompression.
